Hospice Care in an Adult Family Home
Hospice care can often be provided in an Adult Family Home when the resident’s needs can be safely supported and coordinated with the hospice team.
The hospice agency and the Adult Family Home have different roles
The hospice agency provides hospice medical services and clinical support. The Adult Family Home continues to provide residential care, personal assistance, medication support as authorized, meals or feeding assistance, repositioning, hygiene care, and day-to-day observation according to the resident’s care plan.
Care needs may increase near end of life
Some residents may become bedbound, need frequent repositioning, require longer feeding sessions, need bed baths, or require more nighttime care. These changes can increase staffing needs and should be discussed openly with the provider.
Questions to ask
- Can the home safely support the resident’s current needs?
- How will the home communicate with the hospice agency?
- Will additional staffing be needed?
- Are there additional care charges?
- How are nighttime needs handled?
Comfort and communication
Families should understand who to call for medication changes, symptom concerns, care-plan changes, and urgent issues. Clear communication between the resident, family, home, hospice agency, and other healthcare professionals is essential.
